       When we come to your home we will provide-

• Free In-Home Assessment-We'll learn more about you about you, your needs, discuss your goals for our care and the services we can provide, address any safety and health concerns, obtain your medical history and information about your daily activities.
  

• Develop Personalized Plan of Care-Once we have gathered the information needed, we will develop a personalized plan of care for you.
  

• Provide In-Home Visits- Our nurse will provide regular visits, assess your ongoing needs, make any changes needed to ensure you're receiving the best care possible. We provide follow-up communication with client/family members and client representatives.
  

• Coordinate and Advocate Services-  Assist in scheduling appointments, transport to medical providers, attend to client's needs during appointments, pick up prescriptions. Follow-ups with family.
  

• Director of Nursing- We have a nurse on call 24 hours per day to evaluate any changes in your health and facilitate emergency room services if needed.
