Medication Management- Medication management is provided by the nurse and Includes:
- Medication inventory and review
- Order, Re-Order Medication, Update medication list
- Fill medication boxes on an ongoing basis
- Prescription and delivery
- Educate clients about their medication and the importance of medication compliance
- Review meds and update any medication changes post doctor's apt, hospital stay..
Personal Care Services- This service includes all of the services offered in Supportive Services but it allows for the additional help needed as listed below. We are flexible and Personal Care Services can be provided for a minimum of 2 hour increments once per week or up to 24 hours per day, 7 days per week ongoing if needed. For client's receiving 24 hour care, our caregivers are awake at all times to assist client during the night as needed. Our goal is to accommodate your needs. We can increase or decrease your level of service as needed.
- Assist with Activities of Daily Living
- Bathing, bed baths and showering
- Personal Hygiene
- Incontinence Care
- Dressing and Grooming
- Ambulation and Transfers
- Medication Reminders
- Facility sitting/Hospital private duty
- Post Hospitalization/Rehab/Surgery
- End of Life Care
Supportive Services-Any or all of the following services can be provided for a 2 to 8 hour period.
- Activities
- Companionship Errands and shopping
- Light housekeeping
- Meal preparation/diet monitoring
- Transportation, appointment escorts
- Scheduling appointments/assist with calendar of appointments

Sleep-Over Care:
Sleep-over care is for client's who are more independent but would like the added security of having a caregiver in the home over night in the event that assistance is needed. This service enables the client to have assistance no more than twice per night. Client's need to have a separate room for caregivers to sleep in such as a guest room. Then the client can call the caregiver using a call monitor if help is needed. This service also includes our caregivers assisting the client with evening needs and assisting the client with morning needs to help the client start their day.
